Community Homelessness Prevention Initiative |
Provided through Community Living Durham North (CLDN) : Wrap around homelessness prevention and housing supports assisting people to secure and maintain housing. CLDN assist people to connect to housing benefits, secure income, and provide connection to health services. This program is funded by the Regional Municipality of Durham. It is unlike their other programs where the person in need does not need to have a disability or qualify under Developmental Services Ontario and rather can self-refer or have another person or agency refer them to our program. A CHPI Outreach Worker can be reached at 905-852-5919. |
